Visocchi, MD A 54-year-old woman with autosomal dominant polycystic kidney disease, who had received a renal transplant 10 years previously, came to our attention for acute low back pain, associated with fecal and urinary incontinence. General examination was unremarkable. Neurologic examination showed a T7 level of hypoesthesia, paraparesis, and Babinski sign bilaterally. Emergency spine MRI revealed acute subdural hematoma at T6-T8 level (figure, A–C), which was surgically evacuated. There was no evidence of vascular malformations intraoperatively and both in early postoperative spinal MRI and delayed spinal angiography. Screening for bleeding disorders was unremarkable. The patient’s neurologic status was normal at the 6-month follow-up. Among the 19 cases with idiopathic spontaneous spinal subdural hematoma reported in the literature,1 no patient had polycystic kidney disease, a condition which has been associated with vascular fragility.2

Spontaneous thoracic ventral spinal subdural hematoma mimicking a tumoral lesion: a case report
INTRODUCTION Spinal subdural hematoma is rare and can cause serious neurological symptoms. Sometimes, idiopathic spinal subdural hematoma can spontaneously occur without any identifiable underlying etiologies. In this report, we present such an uncommon case of paraplegia caused by idiopathic spinal subdural hematoma that was successfully managed by laminectomy. CASE PRESENTATION A 45-year-ol...
متن کاملIdiopathic Lumbosacral Spinal Chronic Subdural Hematoma
Reports of spontaneous spinal chronic subdural hematoma (SDH) are extremely rare. In the limited cases reported, the origin, symptoms and treatments are diversely revealed. Therefore accurate diagnoses and proper treatments are difficult to determine. We report a 74-year-old male presented with severe low back pain and both buttocks pain. Magnetic resonance imaging showed a diffuse subdural inf...
متن کاملSpontaneous Spinal Subdural Hematoma with Simultaneous Cranial Subarachnoid Hemorrhage
Spontaneous spinal subdural hematoma is reported at a rare level of incidence, and is frequently associated with underlying coagulopathy or those receiving anticoagulant or antiplatelet agents; some cases accompany concomitant intracranial hemorrhage. The spontaneous development of spinal subdural hemorrhage (SDH) is a neurological emergency; therefore, early diagnosis, the discontinuation of a...
متن کاملTreatment of Spontaneous Cervical Spinal Subdural Hematoma with Methylprednisolone Pulse Therapy
We report herein a case of hyperacute onset of spontaneous cervical spinal subdural hematoma treated with methylprednisolone pulse therapy that showed good results. A 57-year-old man was admitted for posterior neck pain and paraparesis which occurred an hour ago. MRI revealed a ventral subdural hematoma distributed from the level of C1 down to T3, compressing the spinal cord. Conservative manag...
متن کامل[Spontaneous spinal subdural hematoma: MRI findings in two cases].
Spontaneous spinal subdural hematoma is an uncommon finding. This report of two cases of spinal subdural hematoma (one hyperacute and one subacute) discusses the current theory of how it develops, the MRI findings, its differentiation from spinal epidural hematoma, and the value of MRI in monitoring patients undergoing conservative treatment. The most significant MRI findings in the hyperacute ...
